Five Months Faster: How Engagement Accelerated a Phase III Outcome
How one global study detected an engagement drop in real time, intervened within days, and moved 741 sites in 30 days.
Recruitment rarely fails suddenly. It drifts. Sites lose sight of study priorities, oversight arrives in episodes, and by the time a screening number flattens on a dashboard, months of site cost have already been spent. The problem is not that study teams lack data. It is that the data they rely on is lagging - it describes what already happened, after the window to act has closed.
A recent global study shows what changes when the signal arrives earlier.
The Signal
Mid-recruitment, site engagement on the study began to trend down. On a conventional dashboard, nothing would have looked wrong for months — screening reports are retrospective, and a slowdown only becomes visible once participants fail to appear. Because engagement was measured continuously at site level, the study team saw the drop as it happened, before recruitment was lost.
That distinction matters. A screening shortfall is a lagging indicator: by the time it is visible, the cost is sunk. An engagement drop is a leading indicator: it is visible while there is still time to respond.
The Intervention
The study team assessed the signal and launched a targeted engagement campaign within days: weekly communications, recruitment drop-in calls, investigator newsletters, and global study updates — each linked directly to current study materials. No new systems were deployed and no additional burden was placed on sites. What changed was cadence, targeting, and relevance, directed by data.
What the Data Shows
The study team plotted every activated site on two axes — engaged or not, screening or not — and compared two snapshots taken 30 days apart, constrained to the same 741 sites so the comparison holds. Within that single month:
- Sites with recent engagement rose from 42% to 59% - a 17 percentage-point gain.
- High performers - sites both engaged and screening - grew from 25% to 35% of the network: 75 additional sites, a 41% increase.
- Low performers - neither engaged nor screening - fell from 27% to 19%.
- Potential performers - engaged but not yet screening - grew from 123 to 181 sites.
Two details in that movement deserve attention. First, the growth in potential performers is not noise. Those sites were reading the protocol, opening communications, and asking questions — they had simply not yet found the right participant. On a screening-only dashboard they are indistinguishable from dead cost. In the engagement data, they are a pipeline.
Second, the increased communication cadence did not fatigue sites. Site sentiment held at 4.9 of 5 across the period, with protocol sentiment at 5.0. Sites do not resent more communication. They resent irrelevant communication.

From signal to control point
Screening results tell you how a site performed last month. Engagement tells you how a site is behaving today - and on this study, the two moved together, with engagement moving first. That timing difference is the operational value. Under-engagement became visible while there was still time to intervene, the intervention could be targeted rather than broadcast, and the effect was measurable within 30 days on the same population of sites.
This is the practical case for digital-first trial execution. It is not about replacing oversight or adding another system to manage. It is about giving study teams a leading indicator they can act on - so that recruitment risk is corrected in weeks, mid-study, rather than discovered at a milestone review when the options have narrowed and the cost has compounded.
